SOUNDS GREAT ENTERTAINMENT
"CONTRACT AGREEMENT"

This is a written agreement between RAY WILLIAMS (d/b/a “Sounds Great Entertainment” hereby referred to as "ENTERTAINER") and _________________ (hereby referred to as "CLIENT").

ENTERTAINER will supply all necessary music and all necessary audio equipment to perform.

SET UP TIME:  Approximately 60 minutes prior to event or as early as possible.

SET UP REQUIREMENTS:  One (1) 6 or 8  foot table plus two (2) electrical outlets.

PAYMENT REQUIREMENTS:  Total fee $_____.  $100 deposit sent to address listed below.Balance of $_______ to be paid by check upon completion of the event.  Payment made payable to:  Ray Williams.

If there are any changes or special requests on behalf of either party prior to the actual event date, verbal confirmation should be made no later than 24 hours of the event.  If a cancellation occurs then a cancellation payment of $100 would be due.

If deposit of $100 is received then no additional fee would be required.

Please make any changes and initial.  If all is in agreement please sign and make a copy for your records.  Then fax, e-mail or ‘snail mail’ a copy back to me.  Thank you!
