Yarn Used

Andean Treasure from Knit Picks
10-11 skeins depending on size.
Size J Hook
The great thing about this pattern is that any yarn or hook can be used, just start with a beginning chain that fits around the forearm and work accordingly.
Pattern as written makes woman's size small.

Notes

All stitches are in between the posts of the dc of the previous round, not in the actual dc.

At the beginning of most rounds, rather than ch 3 for first dc, I do a sc and ch 2. I think it looks better, leaving less of a gap.

For a larger size:
Start with a longer chain and work accordingly; and/or
Repeat Round 42 of the back for a wider back; and/or
Work more rounds of the collar.

Pattern

Chain 30. Join in beginning chain making sure not to twist.
Rnd 1: Ch 3, dc in each ch around. Join to top chain of beginning ch 3.
Rnd 2: Turn. Sc between 1st and 2nd dc, ch 2 (counts as first dc, now and throughout). dc in between next and all dc. Join in top chain. (30 dc)
Rnd 3-8: Turn. Repeat Rnd 2.
Rnd 9: Repeat Rnd 2 increasing 2 dc in the round. (32 dc)
Rnd 10-12: Repeat Rnd 2.
Rnd 13: Repeat Rnd 9. (34 dc)
Rnd 14-16: Repeat Rnd 2.
Rnd 17: Repeat Rnd 9. (36 dc)
Rnd 18-20: Repeat Rnd 2.
Rnd 21: Repeat Rnd 9. (38 dc)
Rnd 22-24: Repeat Rnd 2.
Rnd 25: Repeat Rnd 9. (40 dc)
Rnd 26-27: Repeat Rnd 2.

Start Back

Rnd 28: Repeat Rnd 2, but do not join at the end of round
Rnd 29: Turn. Sc, ch 2 in between 1st and 2nd dc, dc in between next and all dc adding 4 dc in the round. Dc in between last 2 dc and in turning ch. (44 dc)
Rnd 30-32: Turn. Sc, ch 2 in between 1st and 2nd dc, dc in between next and all dc, to end of round, dc in turning chain
Rnd 33: Repeat Rnd 29. (48 dc)
Rnd 34-36: Repeat Rnd 30.
Rnd 37: Repeat Rnd 29. (52 dc)
Rnd 38-40: Repeat Rnd 30.
Rnd 41: Repeat Rnd 29. (56 dc)
Rnd 42-46: Repeat Rnd 30.
Rnd 47: Turn. Sc, ch 2 in between 1st and 2nd dc, work dc in between all dc, decreasing 4 stitches in round. (52 dc)
Rnd 48-50: Turn. Work one round even.
Rnd 51: Repeat Rnd 47. (48 dc)
Rnd 52-54: Turn. Work one round even.
Rnd 55: Repeat Rnd 47. (44 dc)
Rnd 56-58: Turn. Work one round even.
Rnd 59: Repeat Rnd 47. (40 dc)
At the end of this round, join in top chain of 1st stitch.

Start 2nd Arm

Rnd 60: Turn. Repeat Rnd 2. (You should have 39 dc)
Rnd 61-62: Repeat Rnd 60.
Rnd 63: Turn. Work round, dec 1 dc. (38 dc)
Rnd 64-66: Repeat Rnd 60.
Rnd 67: Turn. Work round, dec 2 dc. (36 dc)
Rnd 68-70: Repeat Rnd 60.
Rnd 71: Repeat Rnd 67. (34 dc)
Rnd 72-74: Repeat Rnd 60.
Rnd 75: Repeat Rnd 67. (32 dc)
Rnd 76-78: Repeat rnd 60.
Rnd 79: Repeat Rnd 67. (30 dc)
Rnd 80-87: Work even rounds.
End of Sleeve. DO NOT FASTEN. DO NOT TURN.

Start Cuff

(Do not turn at the end of these rounds)
Rnd 1: (Sc, ch 3, dc) in next space (V-stitch made), *sk next sp, (dc, ch1, dc) in next sp* to end of round. Join in ch 2 of 1st stitch.
Rnd 2-3: (Sc, ch 3, dc) in next V-stitch, *(dc, ch 1 dc) in next V-stitch* to end of round. Join.
Rnd 4: (Sc, ch 2, dc, ch 1, 2 dc) in next V-stitch, *(2 dc, ch 1, 2dc) in next V-stitch*. Join.
Rnd 5: sl st into next dc. (sc, ch 4, dc) in next ch 1 sp, *(dc, ch 2, dc) in next ch 1 sp* to end of round. Join.
Rnd 6-7: (Sc, ch 4, dc) in next V-stitch, *(dc, ch 2 dc) in next V-stitch* to end of round. Join.
Rnd 8: (Sc, ch 2, dc, ch 2, 2 dc) in next V-stitch, *(2 dc, ch 2, 2dc) in next V-stitch*. Join.
Fasten off.

Second Cuff

Join in between any 2 dc of beginning round and follow Rnd 1-8 of first cuff.

Collar

Join with sc along collar at the end of any rnd
Rnd 1: (ch 3, dc) in next space (V-stitch made), *(dc, ch 1, dc) in next space* to end of round. Join. Turn.
Rnd 2: (sc, ch 3, dc) in V-stitch, *(dc, ch 1, dc) in next V-stitch* to end of round. Join. Turn.
Rnd 3: (sc, ch 2, dc, ch 1, 2 dc) in next V-stitch, *(2dc, ch 1, 2dc) in next V-stitch* to end of round. Join. Turn.
Rnd 4: sl st into 1st 2 dc. (sc, ch5 dc) in next ch 1 sp, *(dc, ch 2, dc) in next ch 1 sp* to end of round. Join. Turn.
Rnd 5: (sc, ch 4, dc) in next V-stitch, *(dc, ch 2, dc) in next V-stitch* to end of round. Join. Turn.
Rnd 6: Repeat round 3.
Rnd 7: sl st in 1st and 2nd dc. (sc, ch 6, dc) in next ch 1 sp, *(dc, ch 3, dc) in next ch 1 sp* to end of round. Join. Turn.

Fasten off. Weave in ends.
